I FOUND A GLITCH!!
It happened in Solitude. So you know by the Winking Skeever that there is a house/shop/etc? In front of it is a pile of wood. I got bored while Bishop was doing a scene and started messing around.
I found a cauldron and lined it up to jump in (because why not?) and I did. and this happened.
